MSCTA在肺血管性病变定性、定位诊断中的应用

目的探讨多层螺旋CT血管成像(MSCTA)技术在肺血管性病变定性、定位诊断中的应用价值。方法 37例临床拟诊肺血管性病变患者行多层MSCTA检查后,运用后处理技术如容积再现(VR)、多平面重建(MPR)、最大密度投影(MIP)等方法进行重建,明确病变的性质及部位。结果 37例患者中3例确诊为肺动脉瘤,4例确诊为肺动静脉畸形,29例确诊为肺动脉栓塞,1例正常。肺动脉瘤及动静脉畸形中,位于左肺上叶2例,右肺下叶2例,左肺下叶3例;肺动脉栓塞中两侧肺动脉主干内见血栓15例,一级分支内见血栓25例,二级分支内见血栓29例,三级分支内见血栓23例。结论多层MSCTA可以准确地对肺动脉瘤及动静脉畸形、肺栓塞做出定性、定位诊断,对肺血管性病变的治疗及预后评估有很大帮助。

Clinical application of MSCTA in pulmonary vascular lesions with qualitative and locative diagnosis

Objective To explore the application of 64-slice spiral CT angiography(MSCTA) in qualitative and locative diagnosis of pulmonary vascular lesions.Methods 64-slice spiral CT examinations were performed in 37 patients with suspected of pulmonary vascular lesion in clinic,VR,MPR and MIP techniques were used in all the images to define the qua lity and location of these lesions.Results Among the 37 cases,there were 3 cases of pulmonary artery aneurysm,4 cases of pulmonary artery malformation,29 cases of pulmonary artery embolism,1 case was normal.Within the pulmonary artery aneurysm and pulmonary artery malformation patients,2 lesions were located in superior lobe of left lung,2 lesions were in inferior lobe of right lung,and 3 lesions were in inferior lobe of left lung.There were 15 patients with main pulmonary artery embo lism,25 patients in pulmonary branchesⅠ,29 patients in pulmonary branchesⅡ,and 23 patients in pulmonary branchesⅢ. Conclusion 64-slice MSCTA can accurately diagnose the pulmonary artery aneurysm,pulmonary artery malformation and pulmonary artery embolism with high quality and location.In addition,MSCTA would be helpful for treating and predicting the prognosis of pulmonary vascular lesions.
