16层螺旋CT在甲状腺乳头状癌的诊断价值

目的：总结甲状腺乳头状癌16层螺旋CT表现，提高其诊断准确性。方法回顾性分析52例经手术病理证实的甲状腺乳头状癌的16层螺旋CT表现。结果大部分甲状腺乳头状癌境界不清楚，瘤周“半岛状”瘤结节强化13例，瘤周“残圈征”14例。合并钙化者28例，其中细颗粒状钙化15例，粗颗粒状钙化8例，混合性钙化5例。52例患者中31例发生颈部淋巴结转移，增强后淋巴结明显强化，囊性变5例，合并壁结节4例。结论甲状腺乳头状癌16层螺旋CT表现有一定的特点，16层螺旋CT对甲状腺乳头状癌的定性、显示肿瘤与周围重要器官的关系方面非常有价值。

The diagnostic value of 16-slice spiral CT for papillary thyroid carcinoma

Objective To analyze and evaluate the 16-slice spiral CT features of papillary thyroid carcinoma. Methods The 16-slice spiral CT findings of 52 cases with papillary thyroid carcinoma proved by pathology and surgery were reviewed retrospectively. Results The tumor margin was often ill-defined. After intravenous contrast ad-ministration, there were 13 cases with peninsular-shaped nodules, 14 cases with incomplete ring, which were visible around the tumors. There were calcifications in 28 tumors including punctuate in 15 cases, granular in 8 cases, and mixed calcifications in 5 cases. Of 52 cases, there were 31 cases with lymph node metastasis, cystic degeneration in 5 cases, intracy nodules in 4 cases. Conclusion By 16-slice spiral CT, papillary thyroid carcinoma have characteristic findings, and 16-slice spiral CT is useful for the diagnosis of papillary thyroid carcinoma and predicting tumor inva-sion to the surrounding organs.
