超微量肝素治疗新生儿硬肿症前后血浆α颗粒膜蛋白—140的变化

摘    要:目的 :观察超微量肝素治疗新生儿硬肿症的疗效及机制。方法 :30例新生儿硬肿症患儿。随机分为肝素治疗组(1 5例 )和治疗对照组 (1 5例 )。在综合治疗基础上 ,肝素治疗组加用肝素 ,每小时腹壁皮下缓慢注射 1次 ,每次每公斤体重 1U(1mg =1 2 5U) ,疗程 1~ 3d。治疗前后检测血浆α颗粒膜蛋白 1 4 0 (GMP 1 4 0 )水平 ,并观察各组患儿症状和体征恢复正常时间。结果 :治疗前 ,新生儿硬肿症各组血浆GMP 1 4 0均明显高于正常组 (P <0 0 1 ) ;治疗后 ,肝素治疗组症状和体征恢复正常时间均比治疗对照组为早 (P <0 0 1 ) ,同时GMP 1 4 0水平仍高于正常组 (P <0 0 5 ) ,与正常组无差别 (P >0 0 5 ) ,治疗对照组患儿GMP 1 4 0水平较治疗对照组显著下降 (P <0 0 5 )。结论 :新生儿硬肿症患儿存在血小板的活化 ,超微量肝素可抑制新生儿硬肿症患儿血小板活化功能的异常而改善症状、缓解疾病的发生发展

Abstract:Objective:To study the therapeutic effect and mechanism of ultramicroheparin in treating neonatal scleredema.Methods:30 cases of neonatal scleredema were randomly divided into two groups:the heparin treated group(15 cases) and control treated group(15 cases).On the basis of synthetic therapy,the patients in heparin treated group were used heparin,1U/kg(1mg=125U),slowly injected under the abdominal skin,one time a hour,one to three days are a course of treatment.15 health neonate were served as normal control group.Results:Before the treatment,the plasma level of platelet granule membrane protein 140(GMP 140) in neonatal scleredema was significantly higher than that in normal control group(P<0 01).Following the treatment,GMP 140 level decreased significantly in heparin treated group compared with that in control treated group(P<0 05).The time that symptoms and signs returned to normal were earlier obviously in heparin treated group than that in control treated group(P<0 01).Conclusion:The finding indicate that there were elevated platelet activation in neonatal scleredema,and ultramicroheparin may inhibit the activation of platelet,therefore improve the symptoms and cut off the development of neonatal scleredema.
