光动力学疗法治疗微静脉畸形的疗效观察

摘    要:目的:分析新型铜蒸气激光为光源的光动力学疗法治疗微静脉畸形的有效性及各种因素对疗效的影响。方法:微静脉畸形患者38例,静脉推注血卟啉单甲醚(HMME)3.5~5.0mg/kg,5min内行铜蒸气激光照射。激光功率密度80~100mW/cm2,光斑直径8~9cm,照射时间20~60min,能量密度140~240 J/cm2。治疗随访2个月以上,分析疗效与微静脉畸形分型的关系。结果:38例患者,总有效率达93.7%,其中达I级疗效即完全治愈者16例,占42.1%,取得明显疗效者(包括Ⅱ级、Ⅲ级)20例,占52.6%。经Spearman相关分析疗效与病变分型有相关关系。结论:铜蒸气激光为光源的光动力学疗法治疗微静脉畸形安全有效,病变类型与治疗效果有关。

Abstract:Objective To analyze the efficacy of new copper vapor laser photodynamic therapy for the treatment of micro-venous malformation and the impact of various factors on the efficacy. Methods There were 38 patients with micro-venous malformation(VM)in the group. Hematoporphyrin monomethyl ether (HMME) injected intravenously 3.5~5.0mg/kg, Copper vapor laser was used within 5 minutes.Laser power density was 80~100mW/cm2, spot diameter was 8~9 cm,irradiation time was 20~60min, energy density was 140~240 J/cm2.The patients were followed up for more than 2 months to analyze the relationship between the efficacy and micro VM (PWS) type. Results The 38 patients effective rate was 93.7% which the level 1 effect was 42.1%,16 cases were completely healing. Level II, III effects was 52.6%, 20 caseswere obvious effects patients. Spearman analysis showed there was corelation between the efficacy and micro-VM type.Conclusion Copper vapor laser photodynamic therapy for micro-VM is safe and effective. The efficacy is corelated with the micro-VM type.
