腹腔镜手术治疗急性阑尾炎并弥漫性腹膜炎的临床分析

目的：探讨腹腔镜手术治疗急性阑尾炎并弥漫性腹膜炎与传统开腹手术的优势。方法：选取我院自2007年1月-2013年8月收治的急性阑尾炎并弥漫性腹膜炎的病例，按手术方式分组，其中腹腔镜手术组（L A组）50例，开腹手术组（O A组）50例。通过回顾性分析，对比两组手术时间、住院时间、下床活动时间、胃肠功能恢复时间、术后腹腔残余感染（盆腔脓肿、肠间脓肿、膈下脓肿）发生率及手术切口感染率。结果：腹腔镜手术组（L A组）的手术时间、住院时间、下床活动时间、胃肠功能恢复时间明显短于开腹手术组（O A组），术后腹腔残余感染发生率及手术切口感染率明显低于开腹手术组（OA组），差异有统计学意义（P＜0．05）。结论：腹腔镜手术对急性阑尾炎并弥漫性腹膜炎的治疗是安全、有效的，且优于传统开腹手术。

Clinical Analysis of Laparoscopic Surgery in Treatment of Acute Appendicitis Complicated with General Peritonitis

Objective ：To investigate the advantages of laparoscopic surgery in treatment of acute appendicitis compli-cated with general peritonitis compared to laparotomy .Methods ：We collected the clinical data of 100 patients who suf-fered from the above diseases from Jan .2007 to Aug .2013 in our hospital .According to the operative method ,they were divided into LA group with treatment of laparoscopic appendectomy （n=50） and OA group with treatment of open surgery （n=50） .We retrospectively analyzed the two ones with operative time ,hospitalized period and the abdominal cavity infection and so on .Results：Compared with OA group ,LA group have less abdominal cavity and postoperative incision infection ,quicker recovery ,earlier postoperative exhaust and shorter hospital stay .Conclusion：Laparoscopic appendectomy is safe and effective .It has an advantage over traditional open surgery .
