超声造影在颈部肿大淋巴结鉴别诊断中的临床研究

目的:探讨超声造影在颈部肿大淋巴结性质鉴别诊断中的应用价值。方法:选取97例颈部淋巴结肿大患者的97个颈部肿大淋巴结进行超声检查。先行常规高频彩色多普勒超声检查,然后进行超声造影,观察淋巴结的超声造影表现,将常规彩色多普勒和超声造影结果与病理结果进行比较分析。结果:超声造影判定颈部肿大淋巴结恶性的敏感度为84%(48/58),特异度87%(33/39),准确率83%(81/97)。常规彩色多普勒判定颈部肿大淋巴结恶性的敏感度为72%(42/58),特异性度41%(16/39),准确率为60%(58/97)。结论:超声造影后的灌注显像特征能为颈部肿大淋巴结的鉴别诊断提供有价值的依据。超声造影对颈淋巴结的鉴别诊断准确率明显高于二维彩色多普勒。

Clinical research of contrast-enhanced ultrasound used in differential diagnosis of cervical tumescent lymphadenopathy

Objective:To investigate the application value of the contrast-enhanced ultrasound in differential diagnosis of the cervical tumescent lymph nodes.Methods: 97 cases with cervical tumescent lymph nodes were inspected and made out diagnosis by 2D color Doppler and contrast-enhanced ultrasound.Then surgical excision guided by ultrasonography and pathological examination of these tumescent lymph nodes were executed.Results: The sensitivity of the CEUS was 84%,the specificity was 87% and the accuracy was 83%.The sensitivity of the conventional techniques was 58%,the specificity was 54% and the accuracy was 61%.Conclusion: The perfusion characteristics of the cervical lymph nodes can provide important value to differential diagnosis of the cervical tumescent lymph nodes.With conventional sonography and contrast-enhanced ultrasound,we can do better in differentiations of the cervical tumescent lymph nodes.
