经骶尾入路骶前间叶组织肿瘤切除28例

目的:探讨经骶尾入路切除骶前腹膜后间叶组织肿瘤的经验及手术技巧。方法:分析1987年1月~2004年6月收治的28例骶前腹膜后间叶组织肿瘤的手术经验,肿瘤上界均在骶2水平以下,平均直径6.8 cm,15例初发,13例为复发,经骶尾入路探查并备切除。结果:23例患者均经骶尾入路完整切除肿瘤,5例术中改为经腹辅助骶尾入路联合切除。本组无手术死亡,2例骶前静脉丛损伤,1例肠瘘,术后无肛门失禁。随访8月~10年,仅1例术后7月复发。结论:对于骶2水平及以下的骶前间叶组织肿瘤,经骶尾切除,是安全有效的手术方法,但术前应充分做好经腹辅助骶尾入路联合切除的准备。

Posterior approach used for resection of presacral tumors

Objective:To evaluate the posterior approach for resection of presacral mesenchymal tissue tumors.(Methods:28) cases of pelvic presacral mesenchymal tissue tumors were extracted and retrospectively analyzed.All 28 patients were prepared for operation by posterior approach,including 15 primary cases and 13 recurrent cases.The diameter of tumors in this study was shorter than 8.8 cm and the CTs showed the superior margin of all tumors was on the level of the second sacrum or lower.(Results:23) cases were completely resected by Posterior approach,whereas 5 cases by combined posterior and anterior(approach).There were no death,2 cases of presacral venous damage,1 case of intestinal fistula and no anus irretention in the study.(Conclusion:Presacral) tumors resection by posterior approach was efficacious and the preparation of combined posterior and anterior approach was necessary.
