FHIT和Caspase-3在大肠腺瘤和腺癌中的表达及临床意义

摘    要:目的 探讨大肠腺瘤和大肠癌中脆性组氨酸三联体 (FHIT)和Caspase -3基因的蛋白表达及其与临床病理指标的可能关系。方法 采用免疫组织化学S -P方法检测 15例正常大肠粘膜、3 9例大肠腺瘤和 62例大肠腺癌标本中FHIT和Caspase -3的表达并分析其与组织学分级、淋巴结转移、Dukes分期以及 5年生存率的关系。结果 在正常大肠粘膜、腺瘤和腺癌中FHIT和Caspase -3的阳性表达率分别为 93 .3 3 % (14 / 15 ) ,86.67% (13 / 15 ) ;61.5 4% (2 4/ 3 9) ,5 6.41% (2 2 / 3 9) ;5 0 .0 0 % (3 1/ 62 ) ,45 .16% (2 8/ 62 ) ;腺瘤和腺癌分别与正常大肠粘膜比较FHIT和Caspase -3的表达差异均有显著性 (P <0 .0 5 )。FHIT的蛋白表达与组织学类型无关 ;而与组织学分级、淋巴结转移、Dukes分期和术后 5年生存率有关。Caspase -3的蛋白表达与大肠癌组织学类型、淋巴结转移和Dukes分期无关 ,而与组织学分级和术后 5年生存率有关。结论 FHIT蛋白可能为一重要的肿瘤抑制基因蛋白。FHIT和Caspase -3基因在大肠癌的发生和演化中都起着重要的作用。可作为大肠癌临床评价肿瘤生物学行为及判断预后的有用指标之一

Abstract:Objective To study the possible relations of genetic protein expressions and clinical pathogenic indicators of FHIT and Caspase-3 in patients with colorectal adenoma and adenocarcinoma. Methods Immunohistochemistry S-P method for testing of FHIT and Caspase-3 expressions in 15 specimens of normal colorectal mucous membrane, 39 specimens of colorectal adenoma and 62 specimens of adenocarcinoma and their relations to histological grades, lymphatic metastasis, Duke's system as well as the 5-year survival rate after operation. Results The positive rates of FHIT and Caspase-3 in normal colorectal mucous membrane, colorectal adenoma and adenocarcinoma were 93.33%(14/15),86.67%(13/15);61.54%(24/39),56.41%(22/39);50.00%(31/62),45.16%(28/62) respectively; There existed significant differences in FHIT and Caspase-3 expressions between colorectal adenoma and adenocarcinoma and normal colorectal mucous membrane respectively (P<0.05). The protein expressions of FHIT had no links with the histological types but with histological grades, lymphatic metastasis, Duke's system and the 5-year survival rate after operation. The protein expressions of caspase-3 had no links with the histological types of colorectal adenoma, lymphatic metastasis or the Duke's system but with histological grades and the 5-year survival rate after operation. Conclusion FHIT protein maybe an important tumor suppressor gene protein and FHIT and caspase-3 played a significant role in the generation and evolution of colorectal adenoma and may be used as one of the useful indicators for clinical assessment of tumor biological behavior and prognosis in patients with colorectal adenoma.
