纤维胃镜检查时心电图的改变

<正> 目前纤维胃镜检查在世界各地医院中已成为常规检查和临床研究的主要部分。由于其操作简便、对病人痛苦较小、比较安全且检查效果较好,因此医生都乐于采用它。一般的印象是,纤维胃镜检查所引起的并发症远较硬式或半屈曲式胃镜所引起的并发症要少得多;然而,Levy等发现其并发症与以前应用硬式胃镜所发生者大致相似。在这些并发症中甚至有因心血管并发症而致死的报道。虽然此等病例为数不多,但考虑到在胃镜检查的病人中,原有心血管疾病者并不少,因此,研究胃镜检查过程中所发生的心血管功能状况的变化,对于预防此并发症的发生具有一定的意义。

ELECTROCARDIOGRAPHIC CHANGES IN FIBEROPTIC GASTROSCOPY

Cardiovascular disorders may be encountered in patients in any age group during fiberoptic gastroscopy, which makes not only slight or transient ECG changs in these patients but also abnormal changes in patients revealing normal ECG.The main factors for these ECG changes are fear of the examniation as well as distension of stomach caused by inflation and maneuvers during gastroscopy .No special treatments are required since these changes are slight and transient. However, electrocardiographic examination should be taken for all the patients who will receive gastroscopy. It must be handled with great care if the ECG changes or the cardiac status of the patient,especially the aged,are severe.For these patients,if there are absolute criteria of gastroscopy,premedication should be given before gastroscopy and ECG monitor must be taken during gastroscopy. However the data permit the conclusion that with sufficient care and some precautionary measures before endoscopy,the complications induced by fiberoptic gastroscopy appear not to be of serious nature. Generally speaking, there is no need to have premedication or ECG monitor before or during gastroscopy.
