| 摘 要： | 目的探讨肝内胆管结石胆肠吻合术后再手术的原因及处理方法.方法对近 14年来 15例胆肠吻合术后再手术的肝内胆管结石患者临床资料进行回顾性分析.结果上次手术后主要病变是复杂的肝内胆管结石、胆道狭窄,再手术以肝叶切除、肝胆管空肠 Roux- en- Y吻合术为主要术式,随访结果疗效满意.结论肝内胆管结石手术病灶未去除、术式选择不当是再手术的主要原因,围手术期处理及选用合理的术式是再手术的要点.

| Abstract: | Objective To explore the causes and patterns of reoperation a fter cholangiojejunostomy for hepatolithiasis. Methods The clinical data of15ca ses who had experienced reoperation in the last14years were analyzed retreospect ively. Results epatolithiasis and bile duct stricture were main reason of reope ration.The reoperation contain hepatic lobectomy and Roux-en-Y hepatojejunostomy mainly and the postoperative follow-up was satisfied. Conclusion The reasons o f reoperation were retained stone and inappropriate operative therapy.Preoperati ve management and appropriate reoperative pattern are the keys. |
