无心跳供者腹腔内脏器联合切取术及肾移植效果

目的:总结无心跳供者腹腔内脏器联合切取术的临床经验及对肾移植效果的影响.方法:回顾性分析1993年4月至2001年7月70例无心跳供者腹腔内脏器联合切取和同期501例单纯双肾联合切取的临床资料及其肾移植随访资料.结果:在无心跳供者腹腔内脏器联合切取术组,8～10分钟取出腹内脏器,其肾移植结果与同期单纯双肾联合切取后的肾移植比较,并发症和移植肾1年存活率等方面无统计学差异(P＜0.05).结论:腹腔内脏器联合切取术方法简便、易于掌握,能同时获取多个腹腔内脏器并维护其功能,有效利用了稀缺的供体器官资源.

AN EN BLOC TECHNIQUE FOR ABDOMINAL ORGAN HARVESTING AND THE EFFECT OF RENAL TRANSPLANTATION

Objective:To sum up the clinical experience of the en bloc total abdominal evisceration technique for abdominal organ harvesting and its effect on renal transplantation.Methods:From April 1993 to July 2001,the clinical data of the en bloc total technique in 70 cases and that of the simple en bloc resection of bilateral cadaveric kidney technique in 501 cases of non-hearted donors at the same time were studied retrospectively.Results:This technique allows removal of all the major organs within 8 to 10 minutes after beginning the donor operation.140 kidneys and 65 livers were available for transplantation and only 16(13.56%) kidneys were delayed graft failure and 2 livers were primary dysfunction after surgery.1-year graft survival and surgical complications of renal transplantation were 88.89% and 5.08% respectively for the en bloc total abdominal evisceration technique's group versus 89.08% and 4.69% respectively for the simple en bloc resection of bilateral cadaveric kidney technique's group.These results were not statistically significantly different(P<0.05).Conclusions:The en bloc total abdominal evisceration is fairly good for organ harvesting and the technique is simpler,easier with minimal complications.
