维持性血液透析患者住院的相关因素分析

目的探讨维持性血液透析患者住院的相关因素。方法对我院82例维持性血液透析患者进行回顾性分析,对住院和非住院的维持性血透患者的血清白蛋白、血红蛋白、血肌酐、血尿素氮等生化指标和平均动脉压、透析时间、每日残余尿量、促红素用量及降压药物的使用等临床指标进行比较;利用单因素分析探讨维持性血液透析患者住院的危险因素。结果两组患者间年龄、性别、透析时间、每日残余尿量、血肌酐、血尿素氮水平、促红素用量及降压药物的使用方面均无统计学差异;血清白蛋白、血红蛋白、平均动脉压在两组之间存在有显著统计学差异(P<0.01)。单因素分析结果显示,低血清白蛋白、低血红蛋白、血压控制不良与维持性血液透析患者住院有关(P<0.001)。结论低血清白蛋白、贫血、血压控制不良为维持性血液透析患者住院的危险因素。

Factors associated with hospitalizaion in maintenance hemodialysis patients

Objective To investigate risk factors for hospitalizaion in maintenance hemodialysis patients.Methods The clinical and biochemical features of 82 maintenance hemodialysis inpatients and outpatients were compared. Risk factors for hospitalization were analysed with single-variable analysis.Results There were no significant differences in age, gender distribution, duration of dialysis, residual urine volume, blood level of urea nitrogrn (BUN) and serum creatinine (Scr), dose of erythropoietin and antihypotensive drugs between inpatients and outpatients. Serum albumin and hemoglobin level were significantly lower in inpatients than in outpatients; Mean arterial pressure(MAP)was significantly higher in inpatients than in outpatients. Single variable analysis showed that low serum albumin, anaemia and poorly-controlled hypertension were associated with hospitalization in maintenance hemodialysis patients.Conclusion Low serum albumin, anaemia and poorly-controlled hypertension are risk factors for hospitalization in maintenance hemodialysis patients.
