小儿肺炎合并心力衰竭发病机制的研究进展

摘    要:心力衰竭是小儿肺炎的常见并发症及重要死亡原因之一,其具体发病机制目前仍未明确。近年来,国内外对肺炎合并心力衰竭的发病机制的研究主要从神经体液因子、细胞、分子、基因水平阐述。大量试验研究证实,肺炎合并心力衰竭时血浆内皮素、一氧化氮、血浆尾加压素Ⅱ、利钠肽等因子可随病程进展而发生变化,同时心功能下降还与心肌细胞病理损害、心肌细胞凋亡、肌球蛋白重链成分的变化、心肌细胞钙代谢障碍及血浆纤维蛋白原Bβ-455G/A基因多态性有关。

Abstract:Heart failure is one of the common complications and important dead reasons in children's pneumonia,but its specific pathogenesis has not clear recently.In recent years,pneumonia complicated by heart failure was investigated by China and foreign countries in neurohumoral factors,cellar,molecular,genetic level.Many experiments confirmed that endothelin,nitric oxide,urotensin Ⅱ,natriuretic peptide and so on can be changed with the progression of pneumonia complicated by heart failure,and the decline in cardiac function was associated with pathological damage and apoptosis in myocyte,composition changes in myosin heavy chain,calcium metabolism disorders in myocyte,and genetic polymorphism of fibrinogen Bβ-455G/A.
