交通事故致严重开放性颅脑损伤的特点及救治

目的：探讨交通事故致严重开放性颅脑损伤的特点及救治．方法：对１９８６年１月～１９９５年１２月收治的３６例严重开放性颅脑交通事故伤的临床资料进行回顾性分析．结果：３６例颅脑交通事故伤占同期收治重型开放性颅脑损伤的５２．１０％．其临床特点为：伤势严重，意识障碍明显，有广泛脑挫裂伤，颅内血肿发生率高，伤口污染严重，合并伤多．结论：救治原则应强调院前急救，保持呼吸道通畅，尽早送至专科医院抢救．早期彻底清创，积极治疗并发症，可降低死亡率，减少残废率．

The Characteristics and Treatment of Severe Open Craniocerebral Injuries Caused by Traffic Accidents

Aim: To explore the characteristics of severe open head injuries in traffic accidents and salvage. Methods : The clinical data of 36 cases of severe open head injuries caused by traffic accidents admitted to our hospital from Jan. 1986 to Dec. 1995 were retrospectively analysed. Results: The clinical characteristics of the 36 cases which held 52.10% of the severe open brain injuries in the same period, were serious wound, confusion and unconsciousness, broad brain contusion and laceration, high incidence of intracranial hemotoma, severe wound contamination and complicated multiple systemic injuries. Conclusion: The main principles should be stressed on pre hospital emergency treatment, keeping airway unobstructed, and transporting the patients to hospitals with specialized neuro surgery as soon as possible, actively preventing complications is also needed.
