尿白蛋白肌酐比值和胱抑素C与冠状动脉病变积分的相关性研究

目的 探讨尿白蛋白肌酐比值（UACR）和胱抑素C（Cys C）水平与冠状动脉病变积分（CAS）的关系.方法 共入选173例冠心病（CHD）患者,均行冠状动脉造影并计算CAS,同时选取健康成人60名为对照组.检查入选对象血压、血糖、血脂、血清糖化血红蛋白、UACR及Cys C水平,比较两组UACR和CysC与冠状动脉病变的相关性.结果 冠心病组的UACR和Cys C明显高于对照组.冠心病组中双支病变组患者UACR和Cys C明显高于单支病变组（P＜0.01）,而多支病变组患者UACR和Cys C显著高于双支病变组（P＜0.01）.UACR和Cys C与ACS积分呈明显的正相关（r分别为0.651,0.649,P均＜0.01）,冠状动脉狭窄支数越多,UACR和Cys C水平越高.结论 UACR和Cys C与冠状动脉积分呈明显的正相关,且随冠状动脉狭窄病变支数的增加呈相应增高趋势,可初步预测冠状动脉狭窄程度.

Relationship between urine albumin-to-creatinine ratio,cystatin C and CAS in CHD patients

Objective To investigate the relationship between urine albumin-to-creatinine ratio（UACR）, cystain C（Cys C） and the coronary artery score（CAS） in coronary heart disease （CHD）. Methods 173 CHD pa- tients were underwent CAG and calculated CAS, and 60 healthy people were chose as control group. The basic metabolism index, blood pressure, plasma level of lipid, glycosylated hemoglobin （HbAlc）, UACR and Cys C were measured. The UACR and Cys C among every group were compared and its relationship with diseased coronary vessel was elvated. Resuits The UACR and Cys C of CHD are significant higher than that in control group, which of double coronary vessels lesion were all higher than single vessel lesion（P〈0.01 ）, and which in muhi-ves- sel lesions group was higher than the double diseased （P〈0.01）. There was obviously positive correlation between UACR Cys C level and ACS （ris 0.651,0.649, P〈0.01 ）, The more stenosed coronary, the higher level of UACR and Cys C were. Conclusion UACR and Cys C level are significantly positively correlate to CAS, increasing of UACR and Cys C are dependon the degree of coronary stenosis. They can preliminary forecast of coronary artery stenosis degree.
