急性脑梗死血清铁蛋白的变化及依达拉奉治疗后的影响

摘    要:目的观察急性脑梗死后SF(serum ferritin)水平的变化及依达拉奉治疗后SF的变化及恢复情况;评价脑梗死后48h SF水平与同期的病损程度(CSS评分)及脑梗死体积的关系;评价不同治疗组急性期SF的水平对预后的影响。方法将48例脑梗死患者随机分为两组,检测SF、CSS评分及脑梗死体积并与正常对照组比较。结果急性脑梗死组各观察点SF水平均较健康对照组增高, 依达拉奉治疗组SF水平上升趋势较常规治疗组慢,第5、7d两组相差明显,较常规治疗组CSS评分第7、14、21d明显增高;常规治疗组急性期(48h 5d 7d/3)SF水平低。脑梗死组48h SF水平与入院时CSS评分呈现正相关,与同期脑梗死的体积也呈正相关。结论 SF可能参与了急性脑梗死缺血后自由基损害的重要病理生理过程,依达拉奉治疗能减少自由基对铁蛋白的破坏,控制急性期的SF 水平,可有效改善患者的预后。

Abstract:Objective To observe the change of serum ferritin(SF) in patients with acute cerebral infarction (ACI) , recovery after therapy with edaravone and evaluate the influence of SF level in acute phase on prognosis. Methods 48 patients of ACI were randomly divided into two groups. SF was dertermined, and China Stroke Scale (CSS scale) was applied to evaluate the extent by processor, then the ischemic volume of ACI was calculated by CT scan or MRL Results SF level was higher in ACI group than that in control group. The increase of SF level was slower in edaravone therapy than that in conventional therapy, and the significant difference appeared on the 5th and 7th day between the two groups. The decrease of CSS scale appears obviously higher in edaravone therapy on 7th, 14th and 21st day, SF level at the 48h in ACI group was positive correlation with the CSS scale and the infarct volume at the same term Conclusion SF may play a role in the pathophysiological of the acute phase of ACI and evaravone therapy can reduce damage of SF by preventing free radical and controlling the level of SF.
