呼吸机相关肺炎病原菌临床研究

摘    要:目的观察呼吸机相关性肺炎病原菌菌株分离及其耐药情况,为临床合理使用抗生素提供依据。方法重症医学科(ICU)3年收治264例机械通气患者,其中176例发生了呼吸机相关性肺炎,对其致病菌及药敏结果进行回顾性调查。结果 176例检出313株致病菌,其中革兰阴性菌占85.9%,革兰阳性菌占14.1%,前5位致病菌分别为是铜绿假单胞菌(84株)、大肠埃希菌(66株)、不动杆菌(54株)、肺炎克雷伯菌(51株)和金黄色葡萄球菌(38株)。药敏结果显示这些菌株耐药现象严重。结论呼吸机相关性肺炎病原菌以革兰阴性菌为主,呈多重耐药。临床上合理使用抗生素至关重要。

Abstract:Objective To investigate ventilator - associated pneumonia (VAP) pathogenic bacteria and drug resistance to provide a basis fur clinical treatment. Methods The data of 264 mechanically ventilated patients treated in ICU in the past three years were collected. Of these patients, pathogenic bacteria and susceptibility test were analyzed in 176 patients with VAP. Results In total of 313 pathogenic bacteria isolates were obtained from 176 patients. Of the isolates, gram - negative bacteria and gram - positive bacteria accounted for 85.9% and 14.1%, respec- tively. The top five pathogenic bacteria were Pseudomonas aeruginosa (84 isolates), Escherichia coli (66 isolates), Acinetobacter (54 isolates), Klebsiella pneumoniae (51 isolates) and Staphylococcus aureus (38 isolates). These isolates exhibited serious drug resistance in susceptibility test. Conclusion The pathogenic bacteria of VAP are mainly gram - negative bacteria and these bacteria are often multiple drug resistant. Ration- al use of antibiotics is very important to prevent the emergence of antibiotic resistance.
