甲状腺微小癌超声诊断

目的分析甲状腺微小癌(TMC)的超声征象,探讨超声对TMC的诊断价值。方法总结分析体检或门诊检查并经手术及病理证实的50例TMC患者的超声表现。观察TMC的结节大小、形态、边界和内部超声图像,有无钙化灶等,观察TMC患者颈部的淋巴结血流状态,有无钙化灶等。结果 TMC的超声声像图表现为大小不等的结节、形态不规整、呈圆形或类圆形,无明显包膜,可见细小毛刺呈麦芒样,超声多呈低回声。有6例病变位置的同侧发现有淋巴结肿大,2例双侧颈部发现有淋巴结肿大,其中经病理证实3例为淋巴结炎,5例证实为癌细胞的淋巴转移,转移的淋巴结内可见"针尖样"点状强回声。结论超声检查能够尽早发现和诊断TMC,对临床上采取及时有效的治疗措施具有非常重要的意义。

Ultrasound diagnosis of thyroid microcarcinoma

Objective To sdudy the ultrasonographic signs of thyroid microcarcinoma（TMC） and discuss the diagnostic value of ultrasound for TMC.Methods Ultrasonographic signs of 50 cases of TMC confirmed by surgery and pathology in our hospital were analyzed.The size,shape,border,calcification and internal ultrasound images of TMC nodule,and the blood flow and calcification of cervical lymph nodes were observed.Results Ultrasonography showed that the nodules found in TMC were round or oval shape with irregular margin.The nodules had no capsule and fine burr could be observed.Most nodules displayed low-echo ultrasound.Cervical lymphadenectasis ipsilateral to the leision was found in 6 cases,bilateral cervical lymphadenectasis was found in 2 cases.Pathological examination indicated that lymph node inflammation in 3 cases and lymph node metastasis in 5 cases.＂Needle tip-like＂ punctate echogenic foci was observed in lymph node metastasis.Conclusion Ultrasonography has important significance for the early detection and diagnosis of TMC,as well as timely and effective treatment of TMC patients.
