腰硬联合麻醉不同麻醉体位对剖宫产产妇及新生儿的综合影响比较

目的 比较腰硬联合麻醉不同麻醉体位对剖宫产产妇及新生儿的综合影响.方法 选取2009年1月至2011年7月行剖宫产134例产妇为研究对象,将其随机分为A组和B组,每组各67例.A组进行卧位下麻醉,B组进行坐位下麻醉.将两组产妇不同时间段的血流动力学指标、麻黄碱总用量、不良反应发生率及新生儿Apgar评分进行对比.结果 B组血流动力学指标波动幅度小于A组,麻黄碱总用量、不良反应发生率也优于A组,均有显著性差异(P<0.05),而两组新生儿不同时间段的Apgar评分则无显著性差异(P＞0.05).结论 坐位下腰硬联合麻醉更适用于剖宫产产妇,但不同麻醉体位对新生儿的影响差异不大.

Comparison of the comprehensive influence of different anesthesia positions of combined spinal-epidural anesthesia on parturient underwent caesarean section and newborn

Objective To compare the comprehensive influence of different anesthesia positions of combined spinal-epidural anesthesia on parturient underwent caesarean section and newborn.Methods One hundred and thirty-four parturients hospitalized in our hospital from January 2009 to July 2011 were selected as research object.The parturients were randomly divided into group A and group B with 67 cases in each group,parturients in group A were given anesthesia at horizontal position,parturients in group B were given anesthesia at sitting position.The indexes of hemodynamics at different times,total dose of ephedrine used,incidence of adverse reactions and Apgar score of newborn in two groups were compared.Results The fluctuation range of hemodynamics in group B were smaller than those in group A,total dose of ephedrine and incidence of adverse reactions were all better than those in group A（all P0.05）.However,Apgar score of newborn at different times had no significant differences between two groups（P0.05）.Conclusion The combined spinal-epidural anesthesia at sitting position is more suitable for parturient underwent caesarean section,but there is no significantly differences for newborn.
