摘    要:目的探讨慢性心力衰竭中医分型与神经内分泌[脑钠肽(BNP)水平]的关系。方法 2009年3月至2011年3月期间80例慢性心力衰竭患者按中医辨证分型,观察不同证型的BNP水平。结果经秩和检验,t=14.68,P>0.05,提示各证型间差异无统计学意义;按照各证型平均秩次,随BNP的降低,证型排列分别为阴竭阳脱型→气虚血瘀型→痰饮阻肺型→心肾阳虚型→阳虚水泛型→气阴两亏型。结论神经内分泌(BNP水平)的检测可作为判定不同证型心力衰竭患者心力衰竭程度的重要指标。

Abstract:Objective To discuss the relationship between syndrome differentiation of traditional Chinese medicine for chronic heart fail- ure and blood level of brain natriuretie peptide ( BNP ). Methods Eighty cases of chronic heart failure during March 2009 to March 2011 were classified according to syndrome differentiation of traditional Chinese medicine, and its relationship with blood level of BNP was observed. Results The rank sum test ( t = 14.68, P 〉0.05) demonstrated that there was no obvious difference between these syndromes. According to mean rank of types of syndromes, the order of syndrome types following the lowering of BNP level was arranged as follows : exhausting of yin and yang→qi dif- ficiency and blood stasrs→phlegm retention and resistant lung→heart - renal yang deficiency→water and yang insufficiency→qi and yin insuffi- ciency. Conclusion The detection for blood level of BNP can be a very important indicator in the process of identifying different degree of heart failure in patients.
