脑室内脑膜瘤的MRI诊断

目的探讨脑室内脑膜瘤MRI特征及病理学表现,以提高对该病的诊断准确性。方法回顾性分析经手术病理证实的18例脑室内脑膜瘤影像学表现。所有病例均行MR平扫加增强扫描。结果 18例中,16例位于侧脑室,2例位于三脑室。脑室内脑膜瘤多表现为圆形、类圆形(11例)或分叶状(7例)肿块。T1WI上呈稍低信号(14例)或低信号(4例);T2WI呈稍高信号(12例)或低信号(6例),9例见不同程度的瘤周水肿。增强扫描多明显均匀强化,5例可见少许斑片状低信号。14例见肿瘤与邻近脉络丛关系密切。结论脑室内的脑膜瘤的MRI表现具有一定的特征性,结合临床资料可提高其诊断准确性。

MRI diagnosis of meningioma in cerebral ventricles

Objective The purpose of this study was to present characteristic MRI findings of meningioma in cerebral ventricles in correlation with pathological examination,so as to avoid misdiagnosis and raise the accuracy of diagnosis.Methods MRI findings in 18 patients with pathologically proved meningioma in cerebral ventricles were analyzed..All of 18 cases were examined by both plain and enhanced MRI scanning.Results Tumors were localized in lateral ventricles in 16 cases and in third ventricle in 2 cases.The figures of tumors were shown as globular（11 cases） or lobulated（7cases） soft-tissue mass.The signals of mass showed as slight low intensity in 14 cases or low intensity in 4 cases on T1WI and slight hyperintensity in 12 cases or low intensity in 6 cases on T2WI.Edema had been seen in various degree in surrounding brain parenchyma in 9 cases.Most masses showed moderately to markedly homogeneous enhancement after injection of intraveneous contrast material,5 lesions showed various hypointensity area in enhanced scanning.Tumors were closely related with the adjacent choroid plexus in 14 cases.Conclusion Meningiomas in cerebral ventricles have characteristic appearance on MRI scanning,and combination with clinical data can provide more comprehensive information and may raise the accuracy of diagnosis for these tumors.
