调强适形放射治疗与常规放疗联合化疗治疗局部晚期鼻咽癌的临床疗效分析

目的:探讨调强适形放射治疗联合化疗(IMRT+CT)与常规放疗联合化疗(RT+CT)治疗局部晚期鼻咽癌的临床效果。方法:将128例经局部晚期鼻咽癌患者患者,随机分为两组,对照组采用常规RT+CT方案治疗,观察组采用IMRT+CT方案治疗,用Kaplan-Meier法分析患者1和2年局部区域控制率、无远处转移生存率和总生存率。结果:对照组1年和2年局部区域控制率、无远处转移生存率、总生存率分别为89.1%、90.6%、89.1%和79.7%、82.8%、79.7%,观察组为100.0%。98.4%、98.4%和98.4%、95.3%、98.4%,观察组2年的局部区域控制率和总生存率优于对照组(P<0.05)。结论:调强适形放射联合化疗治疗治疗局部晚期鼻咽癌患者的局部区域控制率和总生存率优于常规的放疗联合化疗,值得临床应用。

Intensity modulated conformal radiation therapy and clinical efficacy of conventional radiotherapy combined with chemotherapy in the treatment of locally advanced nasopharyngeal carcinoma

Objective To investigate the effect of intensity modulated conformal radiation therapy combined with chemotherapy(IMRT+CT) and conventional radiotherapy combined with chemotherapy(RT+CT) in the treatment of local advanced nasopharyngeal carcinoma(NPC).Methods 128 local advanced NPC patients were divided into observation group and control group randomly,which were treated by IMRT+CT and RT+CT respectively.One and two-year local control rates,distant metastasis-free survivals and overall survivals were analyzed by the method of Kaplan-Meier.Results The one and two-year local control rates,distant metastasis-free survivals and the overall survivals were 89.1%、90.6%、 89.1% and 79.7%、82.8%、79.7% in observation group,while 100.0%、98.4%、98.4% and 98.4%、95.3%、98.4% in control group.The twoyear local control rate and the overall survival in observation group were higher than those in control group(P<0.05).Conclusion The two-year local control rate and overall survival in observation group was higher than those in control group in treating local advanced NPC patients which was worthy of clinical application.
