达英-35对非肥胖型PCOS患者内分泌代谢的影响

目的:探讨达英-35对非肥胖型多囊卵巢综合征(PCOS)患者胰岛素敏感性的影响。方法:2006～2007年在本院妇科内分泌门诊就诊的非肥胖型PCOS患者(BMI<25),予达英-35连续治疗6个周期。于治疗前记录体重指数(BMI)、腰臀比(WHR)、多毛评分(F-G评分)和痤疮评分,于早卵泡期测定血清总睾酮(T)、黄体生成素(LH)、卵泡刺激素(FSH)、LH/FSH比值、空腹胰岛素FINS、HOMA-IR。分别于治疗3个月后、6个月后观察这些指标的变化。结果:共41例非肥胖PCOS患者完成了本研究,平均年龄(22.7±3.6)(18～31)岁,平均BMI(21.14±0.65)(18.59～24.20)。治疗期间患者BMI和WHR无显著性变化,F-G评分、痤疮评分、血清T、LH水平和LF/FSH比值均显著降低(P<0.001)。治疗6个月后FINS和HOMA-IR没有明显改变(P>0.05)。结论:非肥胖型PCOS患者短期应用达英-35可明显改善高雄激素血症,而不影响胰岛素敏感性。

Insulin sensitivity during Diane -35 treatment in non -obese women with polycystic ovary syndrome

Objective:To investigate the impact of Diane-35 on insulin sensitivity of non-obese women with polycystic ovary syndrome(PCOS).Methods:Forty-one non-obese(BMI<25) patients with PCOS were assigned to the 6-month treatment with Diane-35.Body mass index(BMI),waist to hip ratio(WHR),F-G score,acne score,serum total testosterone(T),follicle stimulating hormone(FSH),luteinizing hormone(LH),LH/FSH,fast plasma glucose(FPG),fast insulin(FINS),HOMA-IR were measured at baseline.Subjects were resumed at the end of 3 and 6-month follow-up periods.Results:The mean age of 41 patients was 22.7±3.6,mean BMI was 21.14±0.65(range from 18.59 to 24.20).No changes were found in BMI and WHR after 6-month treatment.Serum levels of total T,LH,and LH/FSH ratio were decreased significantly,along with a significant improvement in F-G score and acne score(P<0.001).At the end of 6-months follow-up period,no significant impact in FINS and HOMA-IR was found(P>0.05).Conclusion:Diane-35 can improve hyperandrogenism in non-obese PCOS patients,but not affect insulin sensitivity.
