体外循环术对婴幼儿先心病术后肺的损伤及川芎嗪的保护作用

目的探讨体外循环（CPB）术对婴幼儿先心病术后肺的损伤及川芎嗪的干预作用。方法40例经CPB术行先心病手术的患儿随机分为两组。实验组术前1h静脉给予川芎嗪预处理，对照组给予同等剂量生理盐水。分别在T1、T2、T3、T4、T5时点，通过ELISA法检测IL-6、IL-8血浆浓度及中性粒细胞（Ne）计数。在T1、T3、T4测定肺静态顺应性（Cstat）、氧合指数（OI）、呼吸指数（RI）。结果实验组IL-6、IL-8的表达较低，Ne计数两组均升高；对照组Cstat、OI较实验组降低明显，RI升高显著，实验组肺组织病理改变较轻微。结论婴幼儿先心病CPB术后肺有损伤，术前应用川芎嗪预处理后对肺损伤具有保护作用。

Lung injury after cardiopulmonary bypass and the preventing effects of ligustrazine in infants with CHD

Objective To study the lung injury and the preventing effects of ligustrazine after cardiopulmonary bypass （CPB） in infants. Methods Preoperative 40 patients were randomized into two groups, the treated group and the control group. Patients in the treated group were given with ligustrazine intravenously at 1 hour before the CPB, and the control pa- tients were given with the same dosage saline. To measure the plasma concentrations of the proinflammatory cytokines such as IL-6, IL-8 and Ne at the time point of T1, T2, T3, T4 and T5 with ELISA. Pulmonary functions （RI, OI, Cstat） at the time of T1, T3 and T4. Result The concentrations of IL-6 and IL-8 had decreased in the treated group, the neutrophil quantity increased after CPB in two groups , Cstat and OI decreased more obvious in the control group, RI of the control group patients increased significantly. The change of pathobiology in lung tissue was lighter in treated group than that in control group. Condusion Pulmonary function may be damaged by CPB for CHD. Application of the ligustrazine before the CPB may reduce the injury of lung.
