颈源性眩晕的临床研究进展

椎动脉型颈椎病在临床上发病多由于椎动脉走形,颈椎生物力学失调。根据生物力学观点,由于关节囊韧带松弛,椎间盘退变,导致颈椎生物力学失稳,小关节紊乱卡压到椎动脉,使椎动脉的走形发生变异狭窄。从解剖的角度可以了解椎动脉走行时周围组织以及周围组织损伤对椎动脉的影响。刺激椎动脉周围神经网络,反射性引起椎动脉痉挛,从而引起大脑供血不足;推拿能够改变椎动脉的狭窄情况,改变血流速度。在操作时,手法要稳、力量要均、切忌粗暴过猛,以免损伤颈部组织。

Cervical vertebral artery disease is mostly caused by vertebral artery out of shape resulted cervical biomechanics disorders.According to the biomechanical point of view,the capsular ligament laxity and degenerative disc,result in cervical spine biomechanical instability,facet joint disorders card down to the vertebral artery,causing vertebral artery out of shape mutated stenosis.From the anatomical point of view it can be understood that the deformed vertebral artery impacts the surrounding tissue,which then influences back on the vertebral artery.Stimulation on the neural network around the vertebral artery,causes reflex spasm of the vertebral artery,resulting in insufficient blood supply to the brain;massage can change vertebral artery stenosis to change the blood flow velocity.In practice,steady manipulation,even distributed force are important,excessive force should be avoided so as not to damage the neck tissue.
