改良经尿道等离子体前列腺剜除术治疗良性前列腺增生118例

目的探讨改良经尿道等离子体前列腺剜除术的疗效。方法 2009年3月～2010年5月采用改良经尿道等离子体前列腺剜除术治疗具有手术指征的118例前列腺增生症,先于1点和11点位在膀胱颈到精阜水平切出深达前列腺包膜的沟,将增生的前列腺腺体分为块,再分块剜除,剜除困难者改为常规切除。结果 118例均顺利完成手术,23例因粘连剜除困难,仅剜除一侧叶,另侧叶行常规电切。术中出血量50～150 ml,平均76.3 ml;手术时间40～110 min,平均68.6 min;切除腺体18～72 g,平均39.7 g。与术前经直肠前列腺B超比较,平均切除率56.3%（39.7%～73.3%）。与术前相比,术后3个月患者残余尿量明显减少（79.2±17.3）ml vs.（6.7±3.0）ml,t=2.356,P=0.027],最大尿流率明显增加（6.1±2.3）ml/s vs.（23.0±4.3）ml/s,t=2.587,P=0.012],IPSS评分明显好转（25.7±3.8）分vs.（6.1±4.8）分,t=2.591,P=0.011]。结论分块剜除法能降低等离子体前列腺剜除术难度,操作简单,疗效满意。

Modified Transurethral Plasmakinetic Enucleation of the Prostate for Benign Prostatic Hyperplasia: Report of 118 Cases
