三种不同根管预备技术在磨牙根管治疗中的应用比较

目的 比较逐步后退法、逐步深入法、平衡力法三种不同根管预备技术在磨牙临床根管治疗中的应用效果.方法 选择门诊上颌第一恒磨牙患急慢性牙髓炎或根尖周炎等需要做根管治疗的病例120人,120颗患牙,按就诊顺序分成三组,每组40人,分别采用逐步深入法、逐步后退法及平衡力法进行根管预备,观测记录根备操作时间和根备过程中的断针、侧穿、台阶形成及后续的根管充填情况.以根备效价分值作为评价根备效率和效果的客观指标.结果 三种根备技术的效价值大小依次是平衡力法、逐步深入法、逐步后退法,但三者之间的差异无统计学意义(尸>0.0 5).结论 使用平衡力法进行磨牙根管预备效率高、效果好、严重并发症少,在临床上可作为磨牙尤其是弯曲根管预备的首选.

A Comparation of three different Preparation Techniques in Root Canal Therapy of Molars

Objective To compare the efficacy of three different enlarging techniques(step-down,step-back and balanced force)in the root canal treatment of molars.Methods One hundred and twenty patients with acute or chronic pulpitis or periapical periodontitis of maxillary first permanent molars were randomly assigned to three equal groups(n = 40 each group).The three group molars'root canals were prepared with step-down technique,step-back technique and balanced force technique respectively,and meanwhile the instrumentation time and the circs of instrument fracture,perforation,ledge and later root canal filling were recorded.The score was regarded as the objective standard of evaluating the efficacy of root canal preparation of each molar.The trial data were analyzed by using the SPSS 11.0 statistic software package.Results The order of the efficacy score of the three root canal preparation techinques is balanced force,step down and step back.But there was no statistically significant difference between the three techniques for maxillary molars(P>0.05).Conclusions A high efficacy and a good effect can be gained by using balanced force technique in root canal preparation of molars with less severe defect.Balanced force techinque can be considered as firest choice in root canal preparation of molars especially curved canals.
