硝酸盐还原试验、DNA线性杂交检测结核分枝杆菌耐药性的比较

摘    要:目的比较生物化学检测方法和基因检测方法在结核分枝杆菌耐药性检测中的优劣,以提高耐药性检测方法的准确度。方法以绝对浓度法为“金标准”,对已知耐药浓度的耐药结核分枝杆菌菌株及敏感株采用硝酸盐还原试验(Nitrate Reducrase Assay,NRA)、DNA线性杂交法(Applicating Line Probe Assay LiPA)检测其对利福平(RFP)、异烟肼(INH)、链霉素(SM)和乙胺丁醇(EMB)的耐药性。结果硝酸盐还原试验与绝对浓度法比较,异烟肼(INH)耐药菌株符合率为77.6%,敏感菌株符合率为86.6%;利福平(RPF)耐药菌株符合率为95.8%,敏感菌株符合率为92%;链霉素(SM)耐药菌株符合率为84.1%,敏感菌株符合率为100%;乙胺丁醇(EMB)耐药菌株符合率为94.1%,敏感菌株符合率为85.7%。DNA线性杂交与绝对浓度法比较,异烟肼(INH)耐药菌株符合率为89.6%,敏感菌株符合率为93.3%;利福平(RPF)耐药菌株符合率为98.6%,敏感菌株符合率为100%;链霉素(SM)耐药菌株符合率为78.3%,敏感菌株符合率为100%;乙胺丁醇(EMB)耐药菌株符合率为88.2%,敏感菌株符合率为100%。硝酸盐还原试验与DNA线性杂交比较,异烟肼(INH)耐药菌株符合率为80.6%,敏感菌株符合率为82.9%;利福平(RPF)耐药菌株符合率为100%,敏感菌株符合率为100%;链霉素(SM)耐药菌株符合率为97.2%,敏感菌株符合率为96.7%;乙胺丁醇(EMB)耐药菌株符合率为100%,敏感菌株符合率为83.5%。结论硝酸盐还原试验和DNA线性杂交法检测匀可作为快速结核分枝杆菌耐药性试验方法,两方法具有一致性。

Abstract:Objective To establish a joint detection method of Mycobacterium tuberculosis drug resistance,and improve the sensitivity and specificity of drug resistance detection methods.methodsTaking absolute concentration method for drug susceptibility testing of Mycobacterium tuberculosis as a “golden standard,” examined the Mycobacterium tuberculosis drug resistance and sensitivity to rifampicin (RFP),isoniazid (INH),streptomycin (SM),and ethambutol (EMB) with Nitrate Reducrase Assay(NRA) and Applicating Line Probe Assay(LiPA).Results Comparing the nitrate reduction test with absolute concentration method,the drug resistance coincident rate of INH was 77.6%,and its sensitivity coincident rate was 86.6%; the drug resistance coincident rate of RFP was 95.8%,and its sensitivity coincident rate was 92%; the drug resistance coincident rate of SM was 84.1%,and its sensitivity coincident rate was 100%; the drug resistance coincident rate of EMB was 94.1%,and its sensitivity coincident rate was 85.7%.Comparing linear DNA hybridization with absolute concentration method,the drug resistance coincident rate of INH was 89.6%,and its sensitivity coincident rate was 93.3%; the drug resistance coincident rate of RFP was 98.6%,and its sensitivity coincident rate was 100%; the drug resistance coincident rate of SM was 78.3%,and its sensitivity coincident rate was 100%; the drug resistance coincident rate of EMB was 88.2%,and its sensitivity coincident rate was 100%.Comparing the nitrate reduction test with linear DNA hybridization,the drug resistance coincident rate of INH was 80.6%,and its sensitivity coincident rate was 82.9%; the drug resistance coincident rate of RFP was 100%,and its sensitivity coincident rate was 100%; the drug resistance coincident rate of SM was 97.2%,and its sensitivity coincident rate was 96.7% ; the drug resistance coincident rate of EMB was 100%,and its sensitivity coincident rate was 83.5%.Conclusion Both nitrate reduction test and linear DNA hybridization can be used as a fast detection method of Mycobacterium tuberculosis drug resistance.They are coincident.
