健脾益气中药配合针刺对重型颅脑损伤气管切开术后患者肺部感染的干预作用

目的：探讨健脾益气中药配合针刺对重型颅脑损伤气管切开术后患者肺部感染干预作用及机制。方法将69例重型颅脑损伤患者随机分为治疗组及对照组，对照组给予常规促醒、营养神经、常规康复及对症支持治疗，治疗组在对照组治疗基础上给予健脾益气中药（补中益气汤）与针刺治疗。观察疗程为1个月，比较2组肺部感染发生率、使用抗生素天数、治疗前后血清白蛋白（ ALB）、GCS评分等。结果最后纳入统计治疗组31例，对照组34例，治疗1个月后2组GCS评分无差别，但均较治疗前改善；2组肺部感染次数、使用抗生素天数、血清白蛋白（ ALB）等指标比较，差异有统计学意义。结论健脾益气中药结合针刺疗法能促进胃肠道吸收功能，维持血清白蛋白水平，减少重型颅脑损伤康复期患者的肺部感染机会，并能加快肺部感染的控制，减少抗生素的应用。

Pulmonary infection on severe craniocerebral injury patients in tracheotomy state by using Chinese Jianpi Yiqi medicine combined with acupuncture

Objective To investigate the intervention effect to the pulmonary infection on severe craniocerebral injury patients( SCI)in tracheotomy state by using Chinese Jianpi Tiqi medicine combined with acupuncture,analyze and discuss the intervention mechanism of traditional Chinese medicine and acupuncture for these patients guiding by the theory of“decubitus impairing Qi”. Methods The 69 cases of severe craniocerebral injury aptients were random-ly divided into treatment group and control group,the control group was given conventional nerve nutrition,awaking, routine rehabilitation and symptomatic treatment,the treatment group in the control group treated with traditional Chi-nese medicine Jianpi Tiqi( Buzhongyiqi Decoction)and acupuncture treatment on the base of control group. The ob-servation period was 1 months,the percentage of pulmonary infection,the number of antibiotics,the number of serum albumin(ALB),GCS score were compared between the two groups. Results Finally,31 cases of the treatment group,34 cases of the control group were included in the statistics,after 1 month of treatment,the GCS score between the two groups has no difference,but both were improved,but the number of pulmonary infection,the number of days of using antibiotics,and serum albumin( ALB )between the two groups have statistical significance. Conclusion Chinese Jianpi Tiqi medicine combined with acupuncture can promote gastrointestinal absorption function,maintain the level of serum albumin,reduce the chances of pulmonary infection,and can speed up the control of pulmonary in-fection,reduce the application of antibiotics in SCI patients in rehabilitation stage.
