摘    要:目的:探究重症呼吸机相关性肺炎(VAP)患者治疗中纤维支气管镜辅助治疗的应用效果以及对患者呼吸力学和炎症因子的影响。方法:抽选80例2018年8月~2019年8月在本院进行治疗的重症VAP患者为研究对象,采用计算机随机法分为两组,对照组患者实施常规吸痰治疗,观察组患者实施纤维支气管镜治疗,对比两组患者治疗前后呼吸力学、炎症因子水平以及治疗效果。结果:观察组患者PIP、WOB及Raw水平低于对照组,Cdyn水平高于对照组,P<0.05;观察组患者IL-6、IL-8和PCT水平低于对照组,P<0.05;观察组治疗有效率高于对照组,P<0.05。结论:重症VAP患者治疗中纤维支气管镜辅助治疗的应用能够有效提升治疗效果,改善患者呼吸力学以及炎性因子异常情况,有效缓解患者痛苦。

Abstract:Objective:Explore the effect of fiberoptic bronchoscopy in the treatment of severe VAP and the effects on respiratory mechanics and inflammatory factors.Methods:80 patients with severe VAP in our hospital from August 2018 to August 2019 were selected as the study objects,randomly divided into two groups,the control group was treated with conventional sputum suction,the study group was treated with fiberoptic bronchoscopy,the respiratory mechanics,inflammatory factor levels and treatment effect of the two groups before and after treatment were compared.Result:The level of PIP,WOB and Raw in the study group was lower than that in the control group,the level of Cdyn was higher than that in the control group,P<0.05;the level of IL-6,IL-8 and PCT in the study group was lower than that in the control group,P<0.05;the effective rate in the study group was higher than that in the control group,P<0.05.Conclusion:Application of fiberbronchoscope in the treatment of severe VAP can improve the curative effect,respiratory mechanics and inflammatory factors,relieve pain.
