高频超声诊断乳腺癌腋窝淋巴结转移的价值

摘    要:目的探讨高频超声在乳腺癌腋窝淋巴结转移诊断中的应用价值。方法79例乳腺癌患者术前均行腋窝淋巴结高频超声检查,观察有无肿大淋巴结,淋巴结形态、大小、数目等二维声像图特征及其血流分布,术后与病理对照分析。结果79例患者中,51例检出淋巴结肿大,敏感性64.5%(51/79),其中可疑淋巴结转移27例,病理证实淋巴结转移25例,敏感性92.6%(25/27)。结论高频超声是检出乳腺癌腋窝淋巴结转移一种简便有效的方法,淋巴结横径(T)〉5mm,纵径/横径比(L/T)〈2,可作为诊断腋窝淋巴结转移的重要参考指标,边缘形态、内部回声、彩色多普勒血流分布等声像图可作为诊断淋巴结转移的参考。

Diagnostic value of axillary lymph nodes metastasis of breast carcinoma by high frequency ultrasonography

Abstract:Objective To evaluate the diagnostic value of high frequency ultrasonography in axillary lymph nodes metastasis of breast carcinoma. Methods Seventy-nine patients with breast carcinomas were examined for axillary lymph nodes by high frequency ultrasonography before operation. The shape size,echogenicity and blood supply on high frequency ultrasonography were recorded and compared with that of pathology after operation. Results Among 79 patients, 51 patients showed turgid axillary lymph nodes on high frequency ultrasonography and the sensitivity was 64.5% (51/79).There were 27 patients with susputive axillary lymph nodes metastasis of breast carcinoma,and 25 of them were confirmed by pathology after operation with a sensitivity of 92.6% (25/27). Conclusion High Frequency ultrasonography is a simple and valuable method to show axillary lymph nodes metastasis of breast cancinomas.An important index for axillary lymph nodes metastasis of breast carcinoma is the transverse of axillariy node (T)>5mm and longitude/transverse (S/L) ratio<2. The shape echogenicity and blood supply of axillary lymph nodes are references for metastasis of breast carcinoma.
