速度向量成像技术评价正常人左心室节段心肌收缩功能的初步研究

摘    要:目的应用速度向量成像技术(VVI)比较静息状态下左心室相同节段的正常心肌纵向和径向的运动特点,评价VVI技术对正常人左心室节段心肌收缩功能的临床应用价值。方法选取健康志愿者30例,采用西门子公司Sequoia 512超声诊断仪,在VVI模式下选取心尖四腔、心尖二腔、心尖左室长轴;二尖瓣水平、乳头肌水平及近心尖水平左室短轴切面,定量分析左室前间隔、前壁、侧壁、下壁、后壁及后间隔的径向运动,以及纵向的基底段、中间段及心尖段等3个分段中正常心肌节段的心肌运动速度(Vs)、应变(S)、应变率(SR)。结果正常人左心室各室壁基底段、中间段和心尖段速度依次递减:基底段>中间段>心尖段,差异有统计学意义(P<0.05);应变、应变率差异无统计学意义(P>0.05)。左心室各室壁二尖瓣水平、乳头肌水平、近心尖部水平径向速度、应变、应变率差异无统计学意义(P>0.05)。结论速度向量成像技术能够定量评价左心室节段收缩功能,为临床评价心脏运动及功能提供新方法。

The preliminary research on evaluate the left ventricular segmental myocardial systolic function in normal subjects by velocity vector imaging

Abstract:Objective To compare left ventricular longitudinal and radial direction movement characteristic, which are of the same segments of normal myocardium under the resting state by velocity vector imaging (VVI). Then evaluate clini- cal practice value of VVI on normal Subjects segmental systolic function. Methods Thirty healthy volunteers were enrolled, and then selected clearly apical four chamber view, apical two chamber view, apical left ventricular long axis view, axis view at papillary level, muscle level and nearly apical level. Quantitative analysis velocity (Vs), strain (S), strain rate (SR) about radial motion of left ventricle anterior septum, anterior, lateral wall, inferior, posterior wall and posterior septal; and longitudinal motion of normal segments from basal segment, middle segment and apical segment. Results From basal segment to apex, the velocity gradient descent as basal segment 〉 medial segment 〉 apical segment( P 〈 0.05 ) ,but the strain rate and strain are the same( P 〉0.05). Radial velocity, strain rate and strain among the different walls of the left ventricle are the same ( P 〉 0. 05 ). Conclusion This research indicates that VVI can be used as quantitatively assess left ventricular segmen- tal systolic function. To provide a new method for clinical evaluation cardiac motion and function.
