三维适形放疗替代腔内后装治疗宫颈癌的近期疗效观察

目的:探讨三维适形放疗(three-dimensional conformal radio therapy,3-DCRT)替代腔内后装治疗宫颈癌的近期疗效及并发症的发生情况.方法:对37例宫颈癌患者行普通外放疗40 Gy完成后,不进行腔内后装治疗,改用3-DCRT 30～36 Gy,使总剂量达到70～76 Gy.结果:近期有效率为97.30％(36/37),1、2、3年生存率分别为91.89％(34/37),81.08％(30/37),64.86％(24/37).直肠反应发生率为24.32％(9/37),膀胱反应发生率为13.51％(5/37),消化道反应发生率21.62％(8/37).结论:3-DCRT治疗宫颈癌近期疗效好,不良反应轻,患者易耐受,且操作易行,照射时间及治疗周期短,尤其适合无法进行腔内治疗患者,值得推广.

Observation on short-term curative effects of three-dimensional conformal radiotherapy replacing intracavitory afterloading radiotherapy in cervical cancer

Objective：To explore the short-term curative effect and complications of three-dimensional conformal radiotherapy（3-DCRT） replacing intracavitory afterloading radiotherapy in cervical cancer.Methods：After completion of ordinary external radiotherapy 40 Gy,37 cases of cervical cancer were performed 3-DCRT 30～36 Gy,total dose 70～76 Gy,without intracavitory afterloading radiotherapy.Results：The short-term effective rate was 97.30%（36/37）.The 1-,2-,3-year survival rates were 91.89%（34/37）,81.08%（30/37）,64.86%（24/37） respectively.The occurrence rates of rectal reaction,bladder reaction and gastrointestinal reaction were 24.32%（9/37）,13.51%（5/37） and 21.62%（8/37） respectively.Conclusion：3-DCRT to treat cervical cancer has better short-term curative effect and light adverse reactions,which is easy tolerated by patients and simple to be operated with short irradiation time and treatment cycle,especially suitable for the patients unable to perform intracavitory radiotherapy and worth to be popularized.
