影响孕妇分娩意愿的因素分析

目的了解孕妇的分娩意愿并探讨其影响因素。方法对妊娠≥32周的216例无妊娠合并症及并发症的初产妇进行分娩意愿的问卷调查,将希望选择阴道分娩及不选择阴道分娩的孕妇分为两组进行其影响因素的单因素及logistic回归分析。结果有68.1%的孕妇明确表示希望阴道分娩,11.1%的孕妇表示希望剖宫产,20.8%不能主动决定分娩方式。孕妇不选择阴道分娩的影响因素为：出生地为北京（OR=3.081）,流产次数≥2次（OR=2.854）,孕前BMI≥25kg/m^2（OR=3.274）,孕期情绪不稳定（OR=1.970）,不了解剖宫产的弊端（OR=3.599）。结论多数孕妇希望能够阴道分娩,部分孕妇表示希望剖宫产。影响孕妇选择分娩方式的原因也是多方面的。应当采取多种有效措施进行预防和控制,并针对影响孕妇分娩意愿的因素进行相关教育以降低社会因素剖宫产。

Analysis on influencing factors of pregnant women''''s preference for delivery

Objective To study influencing factors related to pregnant women＇s preference for delivery. Methods Questionnaire survey was conducted among 216 pregnant women with 32 weeks of pregnancy or longer, without medical complications. These pregnant women were divided into two groups, one with preference for vaginal delivery, and the other one with no preference for vaginal delivery, single factor analysis and logistic analysis were used in comparing influencing factors between these two groups. Results 68.1% of the subject chose vaginal delivery, 11.1% definitely chose cesarean section, and 20.8 % could not decide by themselves. Factors of preference for cesarean section included born in Belling （OR= 3.081）, abortions twice or more （OR = 2.854）, prepregnant BMI≥25kg/m^2 （OR = 3.274）, emotional instability during pregnancy （OR = 1.970）, knowing little about disadvantage of cesarean section （OR = 3.599 ）. Conclusion Most pregnant women preferred to vaginal delivery, while some expected cesarean section. Health education should be conducted among pregnant women, based on the influencing factors of preference for delivery.
