DTI定量评价陈旧心肌梗塞病人二尖瓣环舒张早期运动速度与左室舒缩功能的关系

目的 :探讨二尖瓣环6个部位舒张早期运动平均峰速度( MA)在评价陈旧心肌梗塞病人左室舒缩功能方面的应用。方法 :应用组织多普勒成像(DTI)技术测定38例陈旧心肌梗塞病人和30例正常人的 MA,用常规超声心动图测定左室舒张末容积(EDV)、收缩末容积(ESV)和射血分数(EF)。对两组受检者的上述参数进行定量对比分析 ,并对心梗病人的 MA与EDV、ESV、EF进行相关分析。结果 :心梗病人的 MA较正常人的显著减低 ,EDV、ESV较正常人的显著增大 ,EF较正常人的显著减低 ,P值均小于0 0001。心梗病人的 MA与EDV、ESV显著负相关 ,P值均小于0 01 ,与EF显著正相关 ,P值小于0 01 ,与年龄弱相关 ,P值大于0 05。结论 : MA是反映左室舒张功能的指标 , MA提供了有关左室收缩晚期到舒张早期功能的重要信息

Quantitative evaluation of the relation between the early diastolic average peak velocity of the mitral annulus and left ventricular diastolic and systolic function using DTI in patients with old myocardial infarction

Objective:To investigate the value of the early diastolic average peak velocity at 6 sites of the mitral annulus(?MA)for evaluating left ventricular diastolic and systolic function in patients with old myocardial infarction(OMI) Method:In 38 patients with OMI and 30 normal subjects,?MAwas measured by DTI and the end diastolic volume(EDV),the end systolic volume(ESV)and the left ejection fraction(EF)were examined with conventional echocardiography.These parameters of two groups subjects were analyzed quantitatively.?MA of patients with OMI was compared with EDV,ESV and EF.Results:In patients with OMI,?MA was significantly lower than that of normal subjects,P<0 0001;EDV and ESV were significantly larger than these of normal subjects,both P<0 0001;EF was significantly lower than that of normal subjects,P<0 0001.In patients with OMI,?MA significantly negatively correlated with EDV and ESV,both P<0 01;?MA significantly positively correlated with EF,P<0 01;?MA was not significantly correlated with age.Conclusion:This study indicates that ?MA is index of left ventricular diastolic function and provides important information regarding left ventricular function in late systole to early diastole.
