呼吸道非发酵菌感染的耐药分析

摘    要:目的研究我院呼吸内科近3年非发酵菌感染耐药状况。方法收集我院呼吸内科分离的呼吸道病原菌中非发酵菌165株作体外药敏试验,对其结果进行分析比较。结果铜绿假单胞菌对二代头孢菌素及一些三代头孢菌素耐药率在65%以上。对哌拉西林/他唑巴坦敏感性较高。鲍曼不动杆菌对二代头孢菌素及一些三代头孢菌素耐药率在60%以上。对亚胺培南的耐药率仅8.8%。嗜麦芽寡养单胞菌大多数抗生素有耐药性,尤其是亚胺培南,对头孢哌酮/舒巴坦敏感性较高。结论非发酵菌感染呈逐年上升趋势,且耐药率高,特别是嗜麦芽寡养单胞菌,在应用抗菌药物治疗时应根据药敏结果和流行特点选择用药,同时应注意加强病房消毒与细菌耐药监测。

Abstract:Objective To study the drug resistance of non-fermentative bacteria in respiratory medicine department of our hospital in last three years. Methods 165 non-fermentative bacteria were isolated from respiratory tract. The results of their drug resistance were analyzed. Results 65% of Pseudomonas aeruginosa were resistant to the second-generation cephalosporins and some third-generation cephalosporin, and sensitive to piperacillin and tazobactam. More than 60% of Acinetobacter baumannii were resistant to the second-generation cephalosporins and some third-generation cephalosporin,only 8.8% were resistant to imipenem. Stenotrophomonas maltophilia were resistant to most drug, sensitive to cefoperazone sulbactam. Conclusions Non-fermentative bacteria infection was gradually rising, and they have high rate of resistance, especially Stenotrophomonas maltophilia,in the application of antibiotics shall be based on the results of drug sensitivity test, should also pay attention to strengthening wards disinfection and bacterial resistance monitoring.
