| 摘 要： | 目的探讨经尿道输尿管开口环切行肾输尿管全长切除术治疗肾盂癌的应用价值。方法在膀胱镜下先将F5输尿管导管插入患侧肾盂并保留,行肾和输尿管上段切除后,输尿管残端结扎固定于输尿管导管上,在尿道外口牵拉输尿管导管,使输尿管内翻套叠进入膀胱,缝合切口。改截石位,经尿道放入电切镜,沿输尿管开口外周约0.5 cm处环形切割,离断输尿管并完整的拖出尿道。结果7例无一例并发出出血、感染、尿瘘。随访6例5-30月,平均17.8月,患侧输尿管走行区及输尿管膀胱开口部未见肿瘤复发。结论此手术方法创伤小、并发症少、效果好,是肾盂癌的有效手术方法之一。

| Abstract: | Objective To study the value of nephrectomy combined with transurethral excision of remaining ureter in the treatment of renal pelvic tumor.Methods From Sept.2001 to Oct.2004,nephrectomy combined with transurethral excision of remaining ureter has been carried out on 7 cases with renal pelvic malignancy.Of them 6 cases were followed up for 5 to 30 months with an average of 17.8 months.Results No surgical complications were found.By cystoscopy,no tumor occurrence was found at the site of excised urethral orifice.Conclusion The modified surgical technique is effective for the treatment of pelvic canner with fewer traumas to the patient and less complications. |
