注意缺陷多动障碍儿童小脑功能缺陷的功能磁共振成像

摘    要:目的 初步观察注意缺陷多动障碍(ADHD)儿童在执行工作记忆任务时小脑和其他脑区激活有无异常.方法 以12例ADHD儿童和12名年龄、性别、利手、受教育程度方面相匹配的正常对照为研究对象,通过运用快速事件相关fMRI及分类n-back工作记忆任务,研究ADHD儿童小脑等主要脑区激活有无改变.结果 ADHD儿童左侧小脑后叶激活较正常对照明显减弱(P<0.01).结论 ADHD儿童在执行n-back工作记忆任务时显示左侧小脑后叶功能低下.小脑功能低下及其所在的高级认知神经回路功能障碍,可能是ADHD儿童行为学异常的神经基础所在.

Abstract:Objective To examine whether there are differential activations of cerebellum and other brain regions for ADHD children in n-back working memory task or not. Methods Twelve ADHD children and twelve controls (matched with age, sex, IQ, handedness and extent of education) underwent rapid event-related functional magnetic resonance imaging (fMRI) when they performed a Categorial n-back Working Memory Task. One sample and two samples t-test were performed for fMRI data respectively intra- and inter-groups with SPM2. Results Compared with healthy children, ADHD children showed weaker activation in left posterior cerebellar lobe. Conclusion The pattern of the activation of the left posterior cerebellar lobe for ADHD children is significantly different from that of controls. This might suggest that there is dysfunction in cerebellum for children with ADHD, and the cerebellar dysfunction might play a critical role in pathophysiology of ADHD.
