经前胸壁入路腔镜甲状腺手术的应用体会

目的:总结经前胸壁入路腔镜甲状腺手术的临床实用性、安全性、美观性及疗效。方法:回顾分析2001年3月至2011年3月为303例甲状腺结节患者行经前胸壁入路腔镜甲状腺手术的临床资料,300例传统开放甲状腺手术(conventional thyroidectomy,CT)患者为对照组,对比分析两组患者手术时间、术中出血量、术后住院时间、美容效果、术后疼痛及并发症情况。结果:两组患者手术时间、术后疼痛程度差异无统计学意义。腔镜组术中出血量、住院时间、术后并发症发生率优于传统组。术后随访,腔镜组美容效果评分明显高于传统组。结论:与传统开放手术相比,经前胸壁入路腔镜甲状腺手术安全、有效,切口美观,更利于患者的术后康复。

Endoscopic thyroidectomy via anterosternal approach inthyroid diseases

Objective：To assess the feasibility and safety of endoscopic thyroidectomy via anterosternal approach（ETAA）.Methods：From Mar.2001 to Mar.2011,a total of 303 patients with thyroid nodules were admitted to our ward,and received ETAA.The clinical and pathologic characteristics of patients,operation method,operation time,intraoperative blood loss,postoperative hospital stay and complications were analyzed.All the data were compared with those of 300 patients who underwent conventional thyroidectomy（CT group）.Results：There were no significant differences in mean operation time and postoperative pain between the 2 groups.ETAA group had more advantages over CT group on blood loss,complication incidence and postoperative hospital stay.Conclusions：Based on our experiences,ETAA is proved to be a safe and effective procedure with a quicker recovery and better cosmetic effect.We believe it is a good alternative of conventional surgery in selected patients with thyroid nodule.
