主动脉内球囊反搏联合急诊PCI在急性心肌梗死合并心源性休克中的应用

目的探讨主动脉内球囊反搏术（IABP）联合急诊PCI在急性心肌梗死合并心源性休克中的临床应用。方法收集从2009年1月至2012年9月AMI合并心源性休克在IABP支持下行急诊PCI手术病例19例，回顾性分析患者的临床特征、冠脉造影及介入治疗情况，观察使用IABP前后患者心率血压变化，IABP相关并发症，住院期间死亡率。结果急性心肌梗死合并心源性休克患者入院后及时在IABP支持下，成功完成急诊PCI手术，患者应用IABP治疗后心率血压均得到明显改善。IABP相关并发症发生率仅4例，严重并发症0例，无术中死亡，住院期问死亡3例，结论AMI合并心源性休克患者尽早进行IABP支持，结合急诊PCI治疗可降低病死率。

Intra aortic balloon counterpulsation combined with emergency PCI in acute myocardial infarction complicated by cardiogenic shock application

Objective To investigate the clinical application of intra-aortic balloon pump （IABP） joint emergency PCI in acute myocardial infarction with cardiogenic shock. Methods From January 2009 to Septem- ber 2012 AMI cardiogenic shock, IABP to support downlink emergency PCI surgical cases, 19 cases retrospectively analyzed the clinical characteristics of the patients, coronary angiography and interventional treatment of the case, observed before and after the use of IABP The patient＇ s heart rate, blood pressure changes, IABP-related complications, hospital mortality. Results Acute myocardial infarction complicated by cardiogenic shock patients after admission to the support of IABP, successfully completed the emergency PCI were treated with IABP after operation, heart rate and blood pressure are improved obviously. IABP related complications occurred only in 4 cases, 0 cases of serious complications, no death during the operation, 3 patients died during hospitalization, Conclusion The patients with AMI complicated by cardiogenic shock early IABP support, combined with emergency PCI treatment can decrease the case fatality rate.
