鼻腔鼻窦异位脑膜瘤的诊断和外科治疗

目的 :探讨鼻腔鼻窦异位脑膜瘤的临床特点、诊断及治疗。方法 :回顾分析鼻腔鼻窦异位脑膜瘤5例的临床资料。结果 :5例中 ,原发性 1例 ,经鼻侧切开切除肿瘤 ,5个月后肿瘤复发 ,带瘤生存 ;继发性 4例 :经额颞大冠状切口入路手术 3例次 ,颅面联合进路 ,上唇中隔蝶窦径路 ,鼻侧切开、枕颈部入路各 1次。术后随访 1年或 1年以上 ,无复发 2例 ,死亡 1例 ,失去联系 1例。结论 :手术是治疗鼻腔鼻窦异位脑膜瘤的有效方法。切除不净是肿瘤复发的重要因素。确诊必须依靠病理组织学及免疫组化

Diagnosis and surgical treatment of nasal sinus ectopic meningioma

Objective:To explore the clinic characteristics and surgical treatment of nasal sinus ectopic meningioma.Method:Five cases were reported and analyzed retrospectively.Result:Nasal lateral incision approach was applied with one case of primary nasal sinus ectopic meningioma which was found recurrence five months later and survived with tumor;Frontotemporal coronal incision was applied 3 times;combined intra-extracranial approach,occipital neck approach,nasal lateral incision approach and transsphenoidal approach were respectively applied one time.These 4 cases were secondary nasal sinus ectopic meningioma.Followed up for one year and more years,no recurrence was found with 2 cases,death was found with 1 case,another one lost follow-up.Conclusion:The surgical treatment is an ideal and effective method in nasal sinus ectopic meningioma.The remants is an important factor for recurrence.The diagnosis depends on biopsy and immunohistochemistry.
