组织细胞性坏死性淋巴结炎中微小病毒B19的检测及其意义

摘    要:目的研究人微小病毒B19感染与组织细胞性坏死性淋巴结炎(histiocytic necrotizing lymphadenitis,HNL)的关系。方法采用双盲法在32例真性HNL患者及13例正常对照淋巴结活检组织中用巢式PCR和免疫组化染色检测B19病毒基因组DNA和病毒蛋白。结果在32例真性HNL患者淋巴结中,B19病毒DNA和病毒蛋白阳性率分别为90·0%和59·4%,而在对照淋巴结中,两者阳性率分别为53·8%和23·1%。统计学分析显示在真性HNL中B19病毒基因组DNA及病毒蛋白的阳性率更高(P<0·05)。结论B19病毒在真性HNL中有更高感染率,推测与真性HNL的发病可能有一定关系。

Abstract:Purpose To investigate if parvovirus B19 infection play a role in the pathogenesis of true histiocytic necrotizing lymphadenitis (HNL). Methods Tissue blocks of lymph nodes from 32 true HNL patients and 13 normal subjects were confirmed for parvovirus B19 infection by nested polymerase chain reaction and immunohistochemical techniques in a double blind manner. Results B19 viral genomic DNA and viral proteins were detected in 90.0% and 59.4% of the true HNL patients by nested polymerase chain reaction and immunohistochemistry, respectively. In control groups, the positive rates were 53.8% and 23.1%, respectively. Statistical analysis indicated that the true HNL patients had higher B19 infection rates than controls (P=0.014 and P=0.027 respectively). Conclusions Lymph nodes from patients with true HNL had higher infection rate with parvovirus B19 than that in control subjects. These data suggest that parvovirus B19 infection may play an important role in the pathogenesis of HNL.
