河南省和山东省抗结核药品管理评价

目的:评价我国抗结核药品供应系统的绩效。方法:河南省和山东省各随机抽取4个地市和20个县,用13个量化指标通过现场问卷调查获得数据。结果:目前使用的6种抗结核药品均属于国家基本药物,有3种属于世界卫生组织基本药物;各个项目最近3次供货每个批号的药品都有质量检验报告,过去1年药品质量检验不合格率为0%,各项目药品招标采购的价格均低于国际价格;一些机构存在药品缺货、过期和账物相符率低的情况,药品库存控制不合理;一些机构未严格执行标准短程化疗方案;县级药品的信息报告准确率比较低。结论:我国建立了严格的药品质量控制体系,但复治涂阳患者药品的库存控制方法需要进一步研究,且应强化药品管理人员的能力,提高药品信息报告的准确性。

Assessment of Anti-TB Drug Management System in He'nan and Shandong Provinces

OBJECTIVE: To evaluate the performance of the anti- TB drug supply system in China. METHODS: Four prefectures and twenty counties were randomly sampled from He'nan and Shandong provinces for the on- the- spot question- naire survey, and data were obtained by investigating 13 quantitative indicators. RESULTS: All the six anti- TB medicines used currently in China are on the National Essential Medicines List, three of which are on WHO Anti- Tuberculosis Essential Medicines List. For all the projects, all batches No. of anti- TB medicines in the last three shipments had quality control test report; percentage of drugs that had no passed the quality control test in the last year reached 0% , and the drug bidding prices in all the projects were lower than the internal prices paid for TB commodities. Problems including out of stock, expired stock and low conformity rate between stock records and physical counts were seen in some TB dispensaries. Stock control of drugs was not reasonable. Some TB dispensaries failed to follow the standard short- course chemotherapy guideline strictly. The percentage of accurate drug management information at county level was low. CONCLUSIONS: China has established a strict drug quality control system. However, the stock control method for drugs used by sputum smear positive retreatment patients remains to be further studied, and the capability construction of drug control personnel should be strengthened to improve the report accuracy of drug information.
