瑞芬太尼在小儿先天性心脏病手术麻醉中的应用

目的:评价瑞芬太尼在小儿心脏手术快通道麻醉效果及术后恢复情况,探讨其应用价值。方法:选择小儿先天性心脏病择期行房间隔缺损或室间隔缺损修补术患儿32例,麻醉分级Ⅰ-Ⅱ级,随机分为瑞芬太尼组和芬太尼组,分别于不同时点记录血流动力学参数,观察术后机械通气时间及拔管时间。结果:瑞芬太尼组较芬太尼组术中各时点血流动力学参数两组无显著差异,但拔管时间及术后在ICU留观时间显著缩短(P<0.05)。结论:瑞芬太尼能保持术中血流动力学的稳定,术后能早期拔管,适用于小儿心脏手术快通道麻醉。

Effect of application of remifentanil in fast track cardiac anesthesia of children congenital heart disease surgery

Objective：To observe the effect of remifentanil used in fast track cardiac anesthesia of pediatric cardiac surgery.Methods： 32 ASA Ⅰ-Ⅱ pediatric patients with atrial septal defect or simple ventricular septal defect were ran-domly divided into two groups： remifentanil group（group R）and fentanyl group（group F）. The hemodynamie changes were recorded at different stage, respectively.The extubation time and duration in ICU were also recorded.Results： There was no significant difference between the two groups on hemodynamic parameters at the same time point （P〉0.05）. Tracheal extubation time in group R was earlier than that in group F（P〈0.05）. Duration in ICU in group R was shorter than that in group F （P〈0.05）.Conclusion： Remifentanil can be safely and effectively used for fast track cardiac anesthesia in children.
