芍药甘草汤加甘麦大枣汤联合生物反馈治疗老年功能性肛门直肠痛的临床观察

目的观察芍药甘草汤加甘麦大枣汤联合生物反馈治疗老年功能性肛门直肠痛的临床疗效,为治疗该病提供方法。方法选择2017年6月-2020年6月在龙华医院肛肠科治疗的老年功能性肛门直肠痛患者60例,随机分为观察组(n=30)和对照组(n=30)。对照组仅采用生物反馈治疗,观察组在与对照组相同治疗基础上加用芍药甘草汤和甘麦大枣汤口服。疗程12周,观察并比较2组患者治疗前后肛门直肠压力、疼痛视觉模拟(VAS)评分及SF-36生活质量评分。结果治疗后,观察组总有效率显著高于对照组(90%vs70%,P<0.05)。2组治疗后的VAS评分及直肠静息压、肛管静息压均较同组治疗前显著降低,且观察组明显低于对照组(P<0.05);2组的SF-36生活质量各维度评分均较治疗前显著升高,且观察组生理机能、生理职能、躯体疼痛、一般健康状况、情感职能、社会功能、精力、精神健康维度评分均高于对照组(P<0.05)。结论芍药甘草汤加甘麦大枣汤联合生物反馈对老年功能性肛门直肠痛的疗效明显优于单独应用生物反馈治疗,可显著提高患者生活质量,具有一定的临床应用价值。

Clinical observation of Shaoyao Gancao decoction plus Ganmai Dazao decoction combined with biofeedback in the treatment of functional anorectal pain in the elderly

Objective To observe the clinical curative efficacy of Shaoyao Gancao decoction plus Ganmai Dazao decoction combined with biofeedback in the treatment of functional anorectal pain in the elderly.Methods Sixty elderly patients with functional anorectal pain treated in the Anorectal Department of Longhua Hospital from June 2017 to June 2020 were selected and randomly divided into observation group(n=30)and control group(n=30).The patients of the control group were treated with biofeedback therapy alone,while the patients of the observation group were given biofeedback therapy combined with Shaoyao Gancao decoction plus Ganmai Dazao decoction.All patients were treated continuously for 12 weeks.Anorectal pressure,visual analogue scale(VAS)score and life quality scores of SF-36 were observed and compared between the two groups before and after treatment.Results Total effective rate(90.00%)of the observation group was obviously higher than that of the control group(70.00%)(P<0.05).After treatment,VAS score,rectal resting pressure and anal rest pressure of the two groups were significantly lower than those of the same group before treatment,and the observation group was significantly lower than those of the control group(P<0.05).After treatment,the scores of SF-36 in each dimension of the two groups were significantly higher than those of the same group before treatment,and the dimension scores of physiological function,physiological function,physical pain,general health status,emotional function,social function,energy,and mental health of the observation group were higher than those of the control group(P<0.05).Conclusion The curative effect of Shaoyao Gancao decoction plus Ganmai Dazao decoction combined with biofeedback on functional anorectal pain in the elderly is significantly better than that of biofeedback alone,which can significantly improve the life quality of patients,and has certain clinical application value.
