个案管理对肿瘤心脏病患者焦虑抑郁及希望水平的影响

目的探讨个案管理对肿瘤心脏病患者焦虑抑郁及希望水平的影响。方法将62例肿瘤心脏病患者按随机数字表法分为两组,每组31例。对照组予以常规治疗护理,观察组予以个案管理,观察6个月。于干预前后采用焦虑自评量表、抑郁自评量表评定焦虑抑郁情绪,采用Herth希望量表评定患者的希望水平。结果干预后观察组焦虑自评量表、抑郁自评量表评分较干预前显著降低(P<0.01),对照组仅抑郁自评量表评分较干预前显著降低(P<0.01),观察组焦虑自评量表、抑郁自评量表评分显著低于对照组(P<0.01)。干预后观察组Herth希望量表总分及对现实和未来的积极态度、与他人保持密切关系维度分较干预前显著升高(P<0.01),对照组仅对现实和未来的积极态度维度分较干预前显著升高(P<0.05),观察组Herth希望量表总分及各维度分均显著高于对照组(P<0.01)。结论个案管理可以缓解肿瘤心脏病患者的焦虑抑郁情绪,提高其希望水平。

The effect of case management on anxiety,depression and hope level of cancer patients with heart disease

Objective To study the effect of case management on anxiety,depression and hope level in cancer patients with heart disease.Methods A total of 62 cancer patients with heart disease were divided into two groups according to the random number table method,with 31 cases in each group.The control group received routine treatment and nursing,and the observation group received case management for 6 months.Before and after the intervention,the self-rating anxiety scale(SAS)and the self-rating depression scale(SDS)were used to assess anxiety and depression,and the Herth hope index(HHI)was used to assess the patient's hope level.Results After the intervention,the SAS and SDS scores of the observation group were significantly lower than those before the intervention(P<0.01).Only the SDS scores of the control group were significantly lower than those before the intervention(P<0.01).The SAS and SDS scores of the observation group were significantly lower than those of the control group(P<0.01).After the intervention,the total score of HHI,the dimension scores of positive attitudes towards reality and close relationships with others in the observation group were significantly higher than those before the intervention(P<0.01).In the control group,only the dimension scores of positive attitudes towards reality and the future were significantly higher than before the intervention(P<0.05).The total HHI score and scores of all dimensions in the observation group were significantly higher than those in the control group(P<0.01).Conclusions Case management can alleviate anxiety and depression in cancer patients with heart disease,and improve their hope level.
