彩超在肝硬化脾动脉栓塞术前后的应用

目的探讨彩色多普勒在肝硬化合并脾功能亢进患者行部分脾动脉栓塞术前及术后的应用.方法 30例肝硬化脾功能亢进患者行部分脾动脉栓塞术,治疗前后应用彩色多普勒检测脾脏大小,门、脾静脉管径及血流动力学变化,并做定量分析和对比.结果脾脏肿大程度、血流量与栓塞面积呈正相关,栓塞后门、脾静脉管径缩小(P＜0.01),平均血流速度降低(P＜0.01),血流量减小(P＜0.01).结论彩色多普勒能对术前病例的选择、栓塞面积大小做出指导,对术后门、脾静脉的血流动力学改变做出客观评价.

Color Doppler Ultrasonography Before and After Therapeutic Splenic Artery Embolization for Liver Cirrhosis Patients with hypersplenism

Objective To investigate the application of color Doppler ultrasonography(CDUS)in liver cirrhosis patients complicated with hypersplenism before and after partial embolization of the splenic artery(PESA).Methods The splenic size,the splenic and portal vein diameter and hemodynamic changes before and after PESA were quantitatively measured and compared by means of CDUS in 30 liver cirrhosis patients complicated with hypersplenism.Results There was positive correlation between the splenic size,the blood flow vs the splenic infarcted area.After PESA,the diameters of splenic and portal portal veins,mean blood velocities and venous volumes vs those before PESA were reduced,all P<0.01.Conclusions CDUS can help for case choice pre-operatively and give guidance for evaluating infarcted area size,and for detecting portal and splenic venous hemodynamics objectively.
