后路内固定治疗胸腰椎骨折术后矫正角度丢失随访分析

摘    要:目的:回顾性分析后路内固定治疗胸腰椎骨折术后脊柱矫正角度丢失的有关原因。方法:以影像学结果作为观察指标,统计本院1990年1月~2004年5月因创伤造成的胸腰椎骨折病例早期行后路内固定术治疗情况,并分析随访的59例患者术中不同的植骨方法,对内固定拆除后脊柱生理曲度丢失的影响:结果:平均随访时间2.5年:6例出现内固定器物断裂,其他病例中有16例于内固定物取出后发生了不同程度的矫正角度丢失:结论:脊柱矫正角度的丢失,是后路内固定术治疗胸腰椎骨折较为常见的并发症。术中行正确的经椎弓根植骨,并适当使用椎体间融合器(TFC),可较好地避免其发生。

Abstract:Objective:To analyse the loss of spinal corrective angle after posterior-instrumentation operations on thoracic-lumbar fractures.Method:The loss of physiologic bend of spine after implants were taken out along with different methods of bone implantation of 59 followed-up patients with thoracic-lumbar fractures treated by posterior instrumentation during Jan.1990 to May.2004 were analysed by standard of image examination.Result:The average followed-up time was 2.5 years.There were 6 patients who had got their implants broken and 16 patients who lost corrective angle in all that did not result in implants rupture.Conclusion:The loss of spinal corrective angle is one of the familiar postoperative complications after posterior instrumentation operations on thoracic-lumbar fractures.To avoid that,appropriate bone implantation and using TFC properly should work.
