序贯机械通气治疗连枷胸呼吸衰竭的临床研究

目的探讨有创一无创序贯机械通气治疗连枷胸呼吸衰竭的方法和疗效，为临床治疗提供参考。方法将我院收治的80例连枷胸呼吸衰竭患者采用随机抽样法平均分为两组，对照组行有创机械通气，治疗组行序贯机械通气。比较两组患者的血气指标以及APACHEII评分。结果治疗组患者总机械通气时间、入住ICU时间、呼吸机相关肺炎患病率和APACHEⅡ评分及血气指标均优于对照组，差异有统计学意义（P〈0．05）。结论早期行无创序贯机械通气治疗连枷胸呼吸衰竭，可显著改善患者的血气指标，增强一般疗效，有利于提高患者的生活质量。

Sequential mechanical ventilation in the treatment of flail chest and respiratory failure

Objective To explore the curative effect of invasive-noninvasive sequential mechanical ventilation in the treatment of flail chest and respiratory failure. Methods Eighty patients with traumatic flail chest and respiratory failure were randomly divided into the study group and the control group. Patients in the control group were treated with invasive mechanical ventilation, while those in the study group were treated with sequential noninvasive mechanical ventilation. The blood gas index and APACHE Ⅱ score were compared between the two groups. Results The total duration of mechanical ventilation, length of stay in the ICU, incidence of ventilator associated pneumonia (VAP), APACHE Ⅱ score and blood gas indexes were significantly superior than those in the control group, with statistically significant difference (P<0.05). Conclusion Sequential non-invasive mechanical ventilation can significantly improve the blood gas index, enhance clinical effect, and improve quality of life in patients with flail chest and respiratory failure.
