SPECT／CT断层融合显像在分化型甲状腺癌^131Ⅰ治疗后的应用价值

目的评价SPECT／CT断层融合显像在分化型甲状腺癌^131Ⅰ治疗后的应用价值。方法分化型甲状腺癌（DTC）术后患者49例，于^131Ⅰ治疗后5-7d行SPECT／CT全身平面显像（WBS）和局部断层融合显像，诊断结果与病理或其他影像检查及临床随访结果对比，分析其灵敏度及准确率。结果平面显像发现异常摄碘疑诊为转移灶40处，灵敏度、准确率分别为76．09％（35／46）、68．63％（35／51）；断层显像发现异常摄碘诊断为转移灶45处，灵敏度、准确率分别为93．48％（43／46）、89．58％（43／48）。结论DTC患者^131Ⅰ治疗后，SPECT／CT断层融合显像诊断转移灶的效能明显优于平面显像，SPECT／CT断层融合显像对DTC术后转移的诊断有较高的应用价值。

Value of fused imaging of SPECT/CT in patients with differentiated thyroid carcinoma after 131I therapy

Objective To explore the clinical value of fused imaging of SPECT/CT in patients with differentiated thyroid carcinoma (DTC) after 131I therapy. Methods 49 patients with DTC accepting 131I therapy after thyroid surgery underwent planar whole-body scan (WBS) and SPECT/CT fused imaging. The diagnostic performance of WBS and SPECT/CT were comparatively analyzed compared with the pathology or other examinations and follow-up results. The sensitivity and the accuracy were calculated and analyzed respectively. Results SPECT/CT fused imaging found the remains of thyroid gland and metastasis lesion’s location,range and numbers accurately. The sensitivity and the accuracy of 131IWBS and SPECT/CT for detecting the metastasis were 76.09%(35/46),68.63%(35/51)and 93.48% (43/46),89.58%(43/48), respectively. Conclusion 131I-SPECT/CT fused imaging has more important diagnostic value than routine nuclear medicine planar imaging for patients with DTC after 131I therapy.
